Medical billing software can often integrate with EHR systems, but buyers should verify the exact product version, data exchanged, direction, monitoring, security, testing, support and total interface cost.
Define the proposed EHR integration precisely
“Integrates with” may mean the systems are sold together, have an established interface, exchange standard files, use an API or require manual import. Ask the vendor to diagram the proposed connection for the named EHR version. Identify which system owns demographics, coverage, appointments, documentation status, codes, charges, claim status, payments and balances.
Separate features available today from custom development or a roadmap. Require written confirmation from every vendor involved.
Map data in both directions
Outbound charge data is only part of the cycle. Determine whether acknowledgments, claim status, payments, adjustments, denials and patient balances return to the EHR or practice-management system. If staff must check two systems, define which queue is authoritative and how duplicate notes are prevented.
Test provider, location, payer and service combinations. Mappings that work for one clinic may fail for another.
Monitor interface exceptions
Every connection needs a report or alert showing failed, delayed and duplicate messages. Assign an owner and support path that includes the EHR vendor, billing vendor and clearinghouse when appropriate. Record ticket history and the final correction.
Reconcile expected encounters or charges to received transactions after launch and after upgrades. A technically active interface can still omit data.
Protect access and PHI
Use individual vendor accounts, role-based permissions, multifactor authentication and audit logs. Review whether the connection copies, stores or merely displays PHI and which subcontractors are involved. The practice should know how access is approved and terminated.

Validate the complete claim loop
Test a normal claim, corrected claim, secondary claim, rejection, denial and remittance exception. Confirm that identifiers remain consistent and that staff can follow the record across systems. Compare report totals and deposits, not just screen appearance.
Document expected results and keep evidence from testing. Repeat critical tests after material configuration or version changes.
Price implementation and ongoing support
Interface cost may include setup, project management, licenses, transaction fees, vendor support and future changes. Ask which party invoices each charge and how new providers or locations affect pricing. Include internal data cleanup and testing time.
Define who maintains mappings and responds when a vendor update breaks the connection. Low setup cost can be misleading without support responsibility.
Plan for downtime and delayed messages
Ask what happens when either system or the interface is unavailable. Staff need a documented way to preserve urgent work, identify transactions created during the interruption and reconcile them after service returns. Blindly resending a batch can create duplicates, while assuming everything caught up can leave encounters unbilled.
The vendor should provide timestamps, message status and a support escalation path. The practice should test reconciliation after a planned outage or controlled test before relying on the process.
Include upgrades in the continuity plan. Identify who receives release notices, schedules interface testing and confirms that daily totals still agree after a change. Keep a small set of representative transactions for repeatable testing. That evidence is more useful than learning about a broken mapping from an aging report weeks later.
Require an EHR-specific demonstration
Have daily users bring representative workflows to the demonstration. Ask the vendor to show how missing documentation, coding questions, failed messages and unposted remittances appear. Confirm data export and downtime procedures.
